The purpose of the initiative is to foster and strengthen community-based solutions to increase access and reduce demand for specialty care for the community’s uninsured/underinsured population. Kaiser Permanente and the California HealthCare foundation are funding 28 community-based coalitions statewide dedicated to developing and strengthening local solutions to these issues. This statewide grant initiative will provide funding in two phases: Planning grants up to $150,000 for 12 months and multi-year implementation grants up to $300,000 per year for the next two years. Through Fresno HCAP’s experience with engagement and on-going participation of community based organizations, we plan to work collaboratively to create a community plan and strategies to accomplish the following: 1) Increase access to specialty care for vulnerable populations, 2) Decrease and improve demand for specialty care and 3) Improve the health care delivery system for the safety net. During the first year planning phase, we will create and submit a comprehensive three-year implementation plan detailing the strategies and activities Fresno County will employ to increase access and/or supply and/or reduce demand for specialty care for our communities’ uninsured/ underinsured population. The first draft is due in August.
